IC-P02	Applicable to an applicant/approved person who is an individual Applicable to positions: shareholder controller pursuant to section 13B(2) or (2A) of the Ordinance or controller within the meaning of section 9(1)(a)(iii)(B) of the Ordinance Purpose: approval for appointment, notify appointment or notify changes of particulars
IC-P03	Applicable to an applicant/approved person which is a body corporate Applicable to positions: shareholder controller pursuant to section 13B(2) or (2A) of the Ordinance, controller within the meaning of section 9 of the Ordinance or director Purpose: approval for appointment, notify appointment or notify changes of particulars
IC-P04	Applicable to position: actuary under section 15AAA(1) of the Ordinance Purpose: approval for appointment or notify changes of particulars
IC-P05	Applicable to an individual or a body corporate Applicable to positions: controller within the meaning of section 9 of the Ordinance, shareholder controller pursuant to section 13B(2) or (2A) of the Ordinance, director or key person in control functions within the meaning of section 13AE(12) of the Ordinance Purpose: to notify appointment or cessation of appointment
IC-P06	Applicable to position: actuary under section 15AAA(1) of the Ordinance Purpose: to notify appointment or cessation of appointment
IC-P07	Applicable to an individual, a partnership or a body corporate Applicable to position: auditor under section 15(1) of the Ordinance Purpose: to notify appointment or cessation of appointment
